Goitre is enlargement of the thyroid gland.
The diffusely enlarged thyroid gland is usually 2-3 times the normal volume and increased blood flow may be manifested by a thrill or bruit.
There are two forms
1. Simple diffuse goitre- Presence between the ages of 15-25 years, often during pregnency, and tends to be noticed by friends and relatives rather than the patient. The goitre is soft and symmetrical, and the thyroid is enlarged to 2 or 3 times normal. There is no tenderness or limphnode enlargement.
2. Multi nodular goitre- Patients with thyroid enlargement with the absence of thyroid disfunction or simple goitre, as young adults may progress to develop nodules. These nodules grow at varing rates and secrets thyroid harmone 'autonomously', there by supressing the growthand function of the rest of the gland.
Multi nodular goitre is usually diagnosed in patients presenting with a large goitre with or without tracheal compression or sudden painful swelling caused by heamorrhage into the nodularsist.
If the goitre is small, no treatment is necessary, but annual thyroid function testing should be arranged so that it may not progress further.
Calcarea Iodata: It is useful at the age of puberty when the patient is susceptible to cold.
Fucus Vesiculosus: Obese, constipated with exophthalmic non-toxic goitre.
Iodium: Swollen painful with emaciation through eating well. gradual increase in the size of the gland.
Phytolacca Decandra: Nodular goitre swollen with heat and inflammation.
